Torticollis, often referred to as “twisted neck,” is a condition commonly discussed in chiropractic circles. It manifests as muscle spasms that cause the neck to pull to one side, affecting both children and adults. While we often hear about torticollis in infants—up to 16% of newborns experience it due to the challenges of birth—adults are not immune. In fact, it’s estimated that 90% of people will experience at least one episode of torticollis in their lifetime, often triggered by injury or trauma.
In most cases, the pain and disability from torticollis resolve within one to two days. However, when symptoms persist, healthcare providers must consider deeper underlying issues, such as neurological conditions like dystonia, which can cause muscle spasms.
A recent case report from the Journal of Contemporary Chiropractic shed light on a particularly interesting instance of torticollis in a 23-year-old female patient. Despite seeking help from over thirty different providers throughout her life, her right-sided torticollis remained untreated effectively, severely impacting her daily activities, from dressing to household chores.
The patient’s journey began with a challenging birth via C-section, followed by two motor vehicle accidents and various sports injuries. Upon examination, the chiropractor conducted a Rib Cage Compression Test and a full chiropractic assessment, discovering significant subluxations in the first through fourth costosternal joints on the right side. These subluxations caused muscle spasms and severely limited her range of motion, leading to pain and disability.
The chiropractic care plan included specific adjustments to both the cervical and thoracic spine, targeting the affected rib areas. Utilising an activator, the chiropractor aimed to correct these subluxations and restore balance to the patient’s musculoskeletal system.
Throughout the treatment, the chiropractor monitored the patient's alignment while she walked. Notably, her alignment shifted as expected, given her long history with torticollis. To aid in recovery, the chiropractor recommended a brace to help maintain rib alignment during strenuous activities.
Remarkably, the patient experienced relief after her very first adjustment. Common complaints such as headaches and restricted range of motion began to resolve rapidly alongside her chiropractic care. While she continues to use a rib belt for support during physical activities, she has not experienced a return of her head tilt or restricted motion.
This case underscores the importance of addressing potential underlying issues—like rib and chest alignment—in treating torticollis. The ribs play a crucial role in our movement and respiratory function, and their relationship with the scalene and costosternal muscles can significantly affect one’s well-being.
